U.S. Passports

Poquoson Public Library is a designated Passport Acceptance Facility.

We process passport applications by appointment only. Walk-ins are not accepted. Please call 757-868-3060 to make an appointment.

We do not process renewals. Visit Renewing a Passport on the State Department website for instructions. We do take renewal passport photos by appointment.

What you’ll need:

1. Proof of U.S Citizenship:

Your evidence must be an original or certified, physical copy. A certified copy is any document that has the seal or stamp of the official issuing authority. You must submit one of the following documents:

• Full validity, undamaged U.S. passport (expired passports are accepted). Full validity means the document is/was valid for 10 years for adults and 5 years for children under 16.
• U.S. birth certificate that meets the following requirements:
• Issued by the city, county, or state of birth
• Lists your full name, date of birth, and place of birth
• Lists your parent(s)' full names
• Has the date filed with registrar's office (must be within one year of birth)
• Has the registrar's signature
• Has the seal or stamp from the city, county, or state which issued it
• Consular Report of Birth Abroad or Certification of Birth
• Certificate of Naturalization or Citizenship
2. Proof of Identity (provide one of the following):
   • Current, valid driver’s license*

     * License must be issued by Virginia; if not, please bring a secondary I.D. in addition to out of state license

   • Military ID
   • Government ID
   • Certificate of Naturalization or Citizenship
   • For minors under the age of 16, each child must appear in person and both parents/legal guardians must present evidence of identity
3. Completed (but not signed) DS-11 form
4. Passport photo (1) - you can have taken at your appointment for $15 or bring with you.

** U.S. Department of State fees may be paid with check or money order only, one check per application

*** Library Execution fees may be paid with cash, check, or money order

If you require faster processing, your application can be expedited for $60, which is added to the regular U.S. Department of State fee. Call 757-868-3060 to schedule an appointment.
